Effective immediately, degree progress forms are now available as part of the SOAR workflow. The DPS Office will not accept any of the paper forms listed below that were initiated by an advisor after September 16, 2020.

Click on “Add” and enter the student ID number and the GEC category and course information for which a sub is being requested. Check to make sure the student information automatically imports into the form.
